The claim documents that must be submitted include:

• Claim submission/notification letter

• Original policy or a photocopy

• Photocopy of the National Identity Card

• Official report detailing the accident that occurred

• Claim Form completed and signed by the attending physician

• If the Insured passes away, include the autopsy report, a photocopy of the death certificate, and witness statements

• If the Insured goes missing, include the accident report and the search termination letter from the authorities, along with a statement from the heirs agreeing to return the benefit if the Insured is found alive

• If the Insured suffers permanent disability, include the medical examination report (Visum) from the doctor and witness statements

• Original receipts for treatment costs

• Other relevant documents related to the settlement of the claim
